A 19-year-old Albanian man has died after falling into a ventilation shaft in an accident described as "completely avoidable" at a construction site in Acton, London.

The victim, Renols Lleshi , was helping to dismantle scaffolding on the roof garden on the 12th floor of a building under construction, in the Ark Soane Academy area on Mill Hill Road.

According to the investigation, as he stepped on a ventilation shaft, its cover collapsed, sending him falling six floors down, killing him. The British Health and Safety Executive (HSE) found that the space had only been covered with a sheet of plasterboard and roofing foam, which were insufficient safety measures.

It was also discovered that routine inspections of the facility did not include the roof garden area, resulting in the risk not being identified and no warning being given to the scaffolding team.

The deceased's family said that the fact that his death was completely avoidable makes the loss even more devastating for them. His father said that while they are grateful for the investigation and prosecution of those responsible, nothing can bring their son back or ease the pain.

The construction company Jerram Falkus Construction Limited, the victim's employer, admitted breaching work at height regulations and was fined £42,200, as well as ordered to pay an additional £2,000 and £5,000 in court costs by the City of London Magistrates' Court.
